Placing Your Surgery Booking

When you are ready to proceed with your booking we request that you firstly complete the online booking form. This is within an Encrypted secure section of our Website to protect your personal information. This can be found on the right of this page (Blue Box)

You are advised to ensure that you read the term and conditions that you are agreeing to when proceeding through this section. Proceeding is acknowledgement that you have read and agreed to all the terms and conditions. This section will ask a series of personal questions and those related to your current medical condition , please ensure that you are truthful and accurate when entering this information as it is a required by the Hospital in determining your eligibility for surgery.

There is specific section for Women undertaking breast surgery to give the surgeon and indication of what size you currently are and what size you would like to be. This is not to say that you final outcome will have to be what you state here. Final choices will be made between yourself and your surgeon at time of consult.

Please note Information entered into the booking form can be manually changed later by your consultant. If you are unsure of the date that you would like surgery please enter close to what you may be thinking and it can be adjusted later. You are given an option to choose your surgeon should you wish to, we can not always guarantee there availability on the dates that you require, however will endeavor to fulfill your surgeon choice. If you are not sure please choose the option “I am undecided or any available”. Once all required information is completed You will then be taken to a live secured Credit Card process system where you will enter your details for the booking payment, this is to engage the services of Destiny Meditravel to commence to prepare and coordinate everything for you. A receipt for this payment will be sent direct to your e-mail address the subject line will
read “Receipt of APPROVED purchase/payment @ EWAY”. The booking fee is for Meditravel services and is not part payment for any surgery or treatments. It is also non-refundable unless you are rejected as a candidate by the hospital or surgeon for your requested surgery. If you close this pop up box and do not enter any payment then your booking will be tagged and unable to be processed.

Once your payment is complete and form is received you will then receive a confirmation e-mail with a copy of all the information that you have submitted.. This e-mail will also have a attachment with instructions on how you should take the photographs required for the surgeon to enable him to provide a photographic recommendation for your chosen surgery. Once you have taken these please e-mail through, as these will be required when we request your surgery appointments so that we can confirm your appointment with the surgeon.
